• New Patient Evaluation and Initiation of Medication Assisted Treatment (Suboxone)​
  • must pass Pre-screening Process prior to initiation
  • must agree to, sign and adhere to program treatment contract

We make every effort to work with your health insurance company to distribute to financial burden so you can get the treatment you need.

​We have payment plan options and will work with you and your financial situation so you can get the treatment you need to prevent relapses.
Suboxone Membership Package Fee:
$150/ month includes:
- copays and processing fees waived for visits
-1 (non suboxone related) sick visit included per month.
- suboxone related follow up visits same day availabilities
- Short notice medication refills and prior authorizations.
